About the RoleThe successful candidate will have a strong background in nursing, with a focus on gerontology. You will be responsible for assessing, planning, implementing, and evaluating resident care, as well as providing education and support to residents and their families. You will also be expected to work collaboratively with the interdisciplinary team to ensure the delivery of high-quality patient care.
Responsibilities- Assess and evaluate resident needs, developing and implementing individualized care plans
- Provide direct resident care, including administering medications, performing treatments, and providing emotional support
- Collaborate with the interdisciplinary team to ensure the delivery of resident-centered care
- Provide education and support to residents and their families
- Participate in quality improvement initiatives to enhance patient care and outcomes
