About the Opportunity
We are seeking a skilled and compassionate Care Coordinator to join our team at Shannex Incorporated. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest level of care and support for our residents.
Key Responsibilities
- Prepare reports, records, statistics, and other data as needed or requested.
- Participate as a member of the interdisciplinary team in team, client, and family meetings.
- Collaborate with clients and family/friends to meet their program expectations and needs.
- Conduct tours of our facilities and interviews prospective clients and/or families through home, hospital, and/or office visits to obtain pre-admission information.
- Coordinate admission, transfer, and discharge of clients in coordination with the interdisciplinary team.
- Prepare funding requests and reports relating to client needs; maintain client records as required.
- Promote a safety culture by taking every reasonable precaution in the circumstances to ensure the health and safety of employees and all others in the workplace.
- Ensure client safety by following and applying all standard operational procedures.
About You
To be successful in this role, you will possess:
- A Licensed Practical Nurse Diploma or other applicable education.
- CPR certification.
- Previous experience working in long-term care or with seniors.
- Knowledge of the principles of human behavior and ability to apply program-solving skills.
- Strong computer literacy skills.
- Ability to communicate effectively and efficiently (both verbally and in writing).
- Knowledge of available community programs and supports.
- Ability to establish and maintain effective, cooperative working relationships with staff, families, clients, and other stakeholders.
