We are seeking a skilled and compassionate Care Coordinator to join our team at Shannex Incorporated. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest level of care and support for our residents.

Key Responsibilities

  • Prepare reports, records, statistics, and other data as needed or requested.
  • Participate as a member of the interdisciplinary team in team, client, and family meetings.
  • Collaborate with clients and family/friends to meet their program expectations and needs.
  • Conduct tours of our facilities and interviews prospective clients and/or families through home, hospital, and/or office visits to obtain pre-admission information.
  • Coordinate admission, transfer, and discharge of clients in coordination with the interdisciplinary team.
  • Prepare funding requests and reports relating to client needs; maintain client records as required.
  • Promote a safety culture by taking every reasonable precaution in the circumstances to ensure the health and safety of employees and all others in the workplace.
  • Ensure client safety by following and applying all standard operational procedures.

About You

To be successful in this role, you will possess:

  • A Licensed Practical Nurse Diploma or other applicable education.
  • CPR certification.
  • Previous experience working in long-term care or with seniors.
  • Knowledge of the principles of human behavior and ability to apply program-solving skills.
  • Strong computer literacy skills.
  • Ability to communicate effectively and efficiently (both verbally and in writing).
  • Knowledge of available community programs and supports.
  • Ability to establish and maintain effective, cooperative working relationships with staff, families, clients, and other stakeholders.
