The Brant Community Healthcare System is seeking a highly skilled Registered Nurse to fill the Diabetes Education Specialist role. This position is a key member of the Diabetes Education team and will provide exceptional care to patients with diabetes.
Key Responsibilities:- Assess patient's educational needs and develop individualized education plans
- Implement education in the most appropriate format, including group and one-on-one sessions
- Evaluate the effectiveness of education and make adjustments as needed
- Collaborate with the Diabetes Education team to provide comprehensive care
- At least 2 years of experience working in a Diabetes Education Clinic setting
- Graduate of a University Nursing Program (BScN)
- Current Basic Cardiac Life Support (BCLS) certification
- Registration (in good standing) with the College of Nurses of Ontario
- Certification in Diabetes Education or actively pursuing Certified Diabetes Educator (CDE) Designation
- Certification in Insulin Pumps or actively pursuing
- Demonstrated ability to provide diabetes education, including management techniques to a variety of adult patients with Type 1 and Type 2 diabetes and those with gestational diabetes
- Current in evidence-based best practices and clinical practice guidelines
- Demonstrated ability to provide diabetes education, including management techniques to children with Type 1 and Type 2 diabetes
- Ability to engage in health promotion activities on and off-site
- Highly developed communication, presentation, and group facilitation skills
- Experienced in the use of insulin pumps and the continuous glucose monitoring system
- Exceptional interpersonal and communication skills with a team focus
- Self-directed individual with the ability to work both independently and in collaboration with other staff members as a part of program planning
- Demonstrated commitment to continuing education and personal development in nursing
- Creative problem-solving abilities with a focus on solutions and patient-friendly approaches
- Demonstrates commitment to exceptional customer service with patients, families, staff, colleagues, and community partners with an awareness of cultural, racial, economic, and social diversity
- Proven critical thinking skills
- Proven organizational and time management skills with the ability to multi-task and set priorities in a rapidly changing environment
- Well-developed negotiation and conflict resolution skills
- Ability to establish positive and proactive work relationships with external partners
- Demonstrated ability to be a coach, teacher, and mentor to colleagues and students
- Demonstrated excellence in patient assessment and care planning
- Reliable individual with an excellent attendance record and ability to work flexible hours during the day and early evening.
