Occupational Therapist

Bloom Care Solutions is a growing Home Health Care company seeking a Part-Time Occupational Therapist to join our team in Kitchener/Waterloo/Cambridge/Brantford and surrounding areas.
We value communication, leadership, and teamwork and are looking for a positive and motivated individual to be part of our team.
Key Responsibilities:
- Evaluate the effectiveness of treatment plans and modify them accordingly
- Complete assessments in a community setting
- Identify the need for beneficial/assistive devices for patients and coordinate their delivery and installation
- Collaborate with other team members and health care professionals as needed
- Ensure that all patient needs are met
Requirements:
- A regulated health professional Occupational Therapist (OT) with a minimum 2 years experience
- Registered and in good standing with the College of Occupational Therapists of Ontario
- ADP Authorizer is preferred but not required
- Great communication skills – must be able to effectively communicate and think critically
- Valid drivers license and willingness to travel in Kitchener/Waterloo/Cambridge/Brantford area
- Strong time management skills, confidentiality, and professionalism
