Key Responsibilities

  • Provide patient-centered care to patients with rehabilitation needs, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Develop and implement effective care plans to promote patient independence and reintegration into community.
  • Provide education and support to patients and families to promote health and well-being.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements

  • Graduate of a recognized School of Nursing.
  • Current certification or eligibility for certification of competence from the College of Nurses of Ontario as a Registered Practical Nurse.
  • Current BCLS required.
  • FIM certification or willingness to complete certification within 6 months of hire.
  • Recent experience with inpatient medical population required.
  • Recent experience with inpatient rehab population and complex care needs population required.
  • Recent knowledge of the philosophy and concepts of geriatric rehabilitative nursing practice.
  • Demonstrates ability to provide patients and families with educational resources.
  • Demonstrates patient and family-centered care.
  • Demonstrates knowledge and ability to establish a therapeutic relationship.
  • Evidence of on-going professional development.
  • Satisfactory attendance and performance required.

Our 3West unit is a 48-bed specialized unit comprised of rehab patients. We are a skilled group of rehabilitation healthcare professionals who strive for excellence in providing compassionate, patient-centered care. Our goal-focused therapy guides our patients in their journey to independence and reintegration into community.
