Job Title: Registered Practical Nurse - Complex Continuing Care

We are seeking a skilled Registered Practical Nurse to join our team in the Continuing Complex Care Unit at The Salvation Army - Toronto Grace Health Centre.

About the Role:

The successful candidate will provide high-quality patient care in a complex patient environment, promoting health and treating health conditions through supportive, preventative, therapeutic, palliative, and rehabilitative means.

Responsibilities:
  • Provide excellent patient and family-centered nursing care in a culturally sensitive manner.
  • Establish caring, therapeutic relationships with patients and their families to attain or maintain optimal functioning.
  • Work collaboratively with the chronic care team to achieve the unit's mission statement.
  • Participate in continuous improvement, learning, and evidence-based practice.
Requirements:
  • Current Certificate of Competence issued by the College of Nurses' of Ontario in good standing.
  • Current Basic Cardiac Life Support (BCLS) Certification.
  • Minimum 1 year of nursing experience, with experience in continuing complex care.
  • Experience working with medically complex patients in acute care hospitals.
  • Ability to work as a member of the chronic care team within the unit's mission statement.
