Baycrest Hospital is seeking a skilled Registered Practical Nurse to join our Complex Continuing Care Unit (5W). This is a permanent part-time position with a competitive hourly pay rate of $37.05 and 30 bi-weekly hours.

Job Summary:

We are looking for a dedicated and compassionate RPN to provide high-quality care to our older patients with complex medical conditions in a non-acute hospital setting. The successful candidate will work collaboratively with our interdisciplinary care team to develop and implement individualized care plans, provide client-centered care, and support the personal, activation, recreation, and spiritual needs of our clients.

Responsibilities:
  • Plan, deliver, and evaluate nursing interventions, including medications and treatments within the RPN's scope of practice.
  • Provide client-centered care within the interdisciplinary care team to meet the personal care, activation, recreation, and spiritual needs of clients.
  • Collaborate with the Registered Nurse to develop and revise individualized care plans.
Qualifications:
  • Current registration in good standing from the College of Nurses of Ontario (CNO) is required.
  • Current Basic Cardiac Life Support (BCLS) certificate, Level C - Automated External Device (AED) is required.
  • Experience in caring for geriatric patients with complex medical care needs is preferred.
  • Psychogeriatric experience, including mental health assessment and intervention, and working with responsive behaviors is preferred.
  • Excellent interpersonal, communication, and problem-solving skills.
  • Basic computer skills, including Meditech, Microsoft Word, and Excel, are preferred.
