About the Job

The Licensed Practical Nurse Team Leader plays a key role in coordinating Home Support Services by supervising Licensed Practical Nurses, Community Health Workers and other unregulated team members. Key responsibilities include overseeing the implementation of client care plans, providing training and evaluation of care provided by staff, and advocating for changes that improve client care.

In collaboration with the interdisciplinary team, the Team Leader performs assessments, makes nursing diagnoses, implements and provides personal and nursing care for clients in a community health setting. This is a CASUAL OPPORTUNITY, which means hours are not guaranteed and may vary. Casual employees are eligible to apply for internal postings including permanent and temporary positions.

Shifts of work will be scheduled days and evenings; rotating from 06:30 to 14:30, 13:00 to 21:00, 14:00 to 22:00, 08:00 to 16:00 hours.
