- Review and implement care plans, ensuring accurate records and timely completion of assignments.
- Utilize technology and communication tools to ensure seamless care coordination and documentation.
- Collaborate with Care Designers to identify opportunities for care improvement and client satisfaction.
- Meet safety standards and practice competently within the scope of your experience and knowledge.
- 1 year of previous home care experience, with a focus on activities of daily living, such as healthy meal preparation, personal care, and transportation.
- Active practice of acute nursing, including wound care, immunization, and chronic disease management.
- Current CPR Certificate and First Aid certification.
- Graduation from a recognized program for Practical Nurses and 1 year of recent related experience in gerontological nursing.
- Negative TB Test completed within the last 2 years.
- Valid driver's license and reliable transportation.
