Job Summary
The Clinical Resource Nurse is a key member of the healthcare team, responsible for providing clinical expertise and leadership in rehabilitation and geriatric settings. This role requires a strong background in nursing, with a focus on patient-centered care and quality improvement.
Key Responsibilities
- Provide clinical expertise and guidance to nursing staff in rehabilitation and geriatric settings
- Develop and implement quality improvement initiatives to enhance patient care and outcomes
- Collaborate with the Patient Care Manager and Professional Practice Clinician to monitor quality metrics and ensure timely patient flow
- Lead interdisciplinary team rounds and facilitate communication among healthcare team members
- Supervise and assist nursing staff, providing administrative support and patient care as needed
- Review rehab referrals, direct admission and discharge, and assign nurses to patients based on skill mix
Qualifications
- Current Registration with the College of Nurses of Ontario required
- BScN preferred
- Previous clinical resource nurse experience preferred
- Minimum of 3-5 years of clinical experience required
- Clinical expertise in a Med/Surg setting required
- Participates on a regular basis in in-service and educational programs/upgrading
- Demonstrated ability to mentor, support, advise and lead others
- Models a high level of professional practice related to clinical and interpersonal skills
- Demonstrates conflict management and conflict prevention and resolution skills
- Superb problem-solving skills
- Excellent communication skills both written and verbal
- Strong organizational skills
- Acts as a professional role model for his/her nursing colleagues
- Actively promotes a healthy workplace for all staff
- Proven success in developing relationships with the interprofessional healthcare team
- Must be able to meet the physical demands of the position
- Satisfactory employment and attendance record required
