Job Description

Job Summary

The Clinical Resource Nurse is a key member of the healthcare team, responsible for providing clinical expertise and leadership in rehabilitation and geriatric settings. This role requires a strong background in nursing, with a focus on patient-centered care and quality improvement.

Key Responsibilities

  • Provide clinical expertise and guidance to nursing staff in rehabilitation and geriatric settings
  • Develop and implement quality improvement initiatives to enhance patient care and outcomes
  • Collaborate with the Patient Care Manager and Professional Practice Clinician to monitor quality metrics and ensure timely patient flow
  • Lead interdisciplinary team rounds and facilitate communication among healthcare team members
  • Supervise and assist nursing staff, providing administrative support and patient care as needed
  • Review rehab referrals, direct admission and discharge, and assign nurses to patients based on skill mix

Qualifications

  • Current Registration with the College of Nurses of Ontario required
  • BScN preferred
  • Previous clinical resource nurse experience preferred
  • Minimum of 3-5 years of clinical experience required
  • Clinical expertise in a Med/Surg setting required
  • Participates on a regular basis in in-service and educational programs/upgrading
  • Demonstrated ability to mentor, support, advise and lead others
  • Models a high level of professional practice related to clinical and interpersonal skills
  • Demonstrates conflict management and conflict prevention and resolution skills
  • Superb problem-solving skills
  • Excellent communication skills both written and verbal
  • Strong organizational skills
  • Acts as a professional role model for his/her nursing colleagues
  • Actively promotes a healthy workplace for all staff
  • Proven success in developing relationships with the interprofessional healthcare team
  • Must be able to meet the physical demands of the position
  • Satisfactory employment and attendance record required
