We are seeking a dedicated Registered Nurse/Registered Psychiatric Nurse to join our Hospice Residence team at Ridge Meadows Hospital. As a member of our multidisciplinary team, you will provide direct nursing care to patients, assessing their physical, emotional, spiritual, and psychosocial needs, and developing and implementing care plans to meet their unique needs.
Responsibilities- Assess patients' needs and develop individualized care plans, taking into account their medical history, current condition, and personal preferences.
- Provide direct nursing care, including administering medications, performing procedures, and monitoring patients' conditions.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
- Participate in interdisciplinary conferences and contribute to the development of patient care plans.
- Document patient information accurately and thoroughly, including progress notes, medication lists, and other relevant data.
- Collaborate with other healthcare professionals to ensure comprehensive care and address any concerns or issues that may arise.
- Stay up-to-date with the latest nursing practices, technologies, and research to provide evidence-based care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote patient safety and satisfaction.
- Graduation from an approved School of Nursing and/or Psychiatric Nursing.
- Two years' recent, related medical nursing experience in an acute care facility, including one year recent related experience in a hospice/palliative care environment.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a multidisciplinary team, with a strong commitment to patient-centered care.
