Indigenous Patient Navigator

We are seeking a dedicated and compassionate Indigenous Patient Navigator to join our team at Interior Health Authority. As a key member of our Integrated Health Outreach team, you will play a vital role in ensuring that Indigenous clients receive culturally safe and client-centered care.
About the Role:The Indigenous Patient Navigator will work closely with healthcare providers, Indigenous clients, and community organizations to identify and address the unique needs of Indigenous clients. This will involve providing consultative and navigational services to support cultural interpretation and health literacy translation, as well as coordinating client referrals and connecting them with community resources.
Key Responsibilities:- Provide consultative and navigational services to healthcare providers and Indigenous clients to support culturally safe care
- Coordinate client referrals and connect them with community resources
- Support Indigenous clients in accessing and navigating healthcare services, including acute care, home health, and community services
- Collaborate with healthcare providers to identify and address cultural safety concerns
- Provide education and training to healthcare providers on cultural safety and Indigenous health
