Qualifications and Requirements

We are seeking a highly skilled and experienced Community Outreach Intensive Case Management Nurse to join our team at Island Health. The successful candidate will possess a strong background in psychiatric nursing, with a minimum of one year's recent experience working with adults with mental disorders.

The ideal candidate will have a solid understanding of the Psychosocial Rehabilitation (PSR) model, as well as assessment and treatment methodologies. They will also be knowledgeable about relevant mental health and addictions legislation, including legal statutory boundaries governing consumer rights.

Key responsibilities will include:

  • Assessing clients' physical, mental, and environmental needs
  • Developing and implementing individualized care plans
  • Providing education and support to clients and their families
  • Collaborating with healthcare teams to ensure comprehensive care

The successful candidate will be a registered nurse with a strong commitment to providing high-quality, patient-centered care. They will also possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment.

Skills and Abilities

  • Ability to communicate clearly and effectively both verbally and in writing
  • Ability to use the nursing process in mental health treatment programs
  • Ability to administer and monitor the side effects of psychotropic medications
  • Ability to work independently in a community setting
