Job Details

At CareRx, we are seeking a skilled Pharmacist (Part-time) to join our team of dedicated healthcare professionals. This part-time position involves 16 hours per week and offers a unique opportunity to work with a leading provider of specialty pharmacy services to seniors.

About the Role:
We are seeking a highly motivated and experienced Pharmacist (Part-time) to ensure the accurate and safe dispensing of medications, while providing exceptional patient care. The successful candidate will be responsible for verifying safe and appropriate medication administration, assessing and dispensing critical medications, and rapidly responding to palliative care prescriptions.

Responsibilities:
• Collaborate with the pharmacy team to ensure medication orders are dispensed accurately, safely, and timely.
• Complete daily tasks in accordance with established workplace policies and procedures.
• Consult with members of the pharmacy management team when resolving issues surrounding day-to-day work functions.
• Provide exceptional customer care to CareRx Pharmacy patients and home partners.
• Maintain accurate, organized, and complete resident health records, ensuring appropriate documentation and continuity of care.
• Conduct medication regimen/profile reviews with associated documentation of recommendations.
• Follow all provincial and federal laws, standards, and regulations governing pharmacy practice.
• Participate in ongoing continuing education, remaining current in pharmaceutical, medical, and professional licensing information.

Requirements:
To be successful in this role, you will require:
• Current licensure and good standing with the Ontario College of Pharmacists.
• Proven ability to foster strong partnerships with other healthcare team members.
• Effective organization, planning, and time management skills.
• Demonstrated high level of fluency in both written and spoken English.
• Knowledge of Kroll Windows application.
• Excellent customer service and interpersonal skills, oral and written communication skills, and ability to interact with persons of varying levels of professional expertise in a professional and mature manner.
• Highly organized and possess excellent time management skills.
• Skilled in critical thought processes and able to prioritize appropriately.
• Must demonstrate initiative and creativity.
• Self-directed requiring minimal supervision and able to work independently.
• Motivated to achieve results that reflect a high standard of quality.
• Must demonstrate good listening skills and be adaptable to changes in job environment.
• Dependable and responsible in the execution of job duties.
• Adaptable to internal work changes.
• Previous long-term care pharmacy experience preferred.
• Strong teaching skills.

Preferred Qualifications:
• Knowledge and experience with Kroll software.
• Knowledge and experience with PacMed.
• Previous long-term care pharmacy experience.
