Care Coordinator for Complex Geriatric Patients

We are seeking a skilled Care Coordinator to join our team at Fraser Health. As a key member of our healthcare team, you will play a vital role in facilitating the discharge process for patients from acute care facilities to community-based services.
About the Role
This is a challenging and rewarding opportunity to work with complex geriatric patients and adults with chronic or acute illnesses. You will be responsible for assessing client needs, coordinating care, and arranging referrals to appropriate community services. Your excellent communication and problem-solving skills will enable you to work effectively with clients, families, healthcare professionals, and other service providers to ensure seamless transitions and optimal outcomes.
Key Responsibilities
* Conduct thorough assessments of client needs and develop effective care plans
* Coordinate with healthcare teams to facilitate timely discharges and referrals to community services
* Liaise with clients, families, and healthcare professionals to provide advice and support
* Manage short-term Home Support hours for clients awaiting discharge or referral
* Participate in quality improvement activities to enhance program effectiveness
Qualifications
We require a Bachelor's degree in Social Work from an approved school, as well as two years' recent experience working with complex geriatric patients and adults with chronic or acute illnesses. Current full registration with the BC College of Social Workers is also essential. Additionally, you should possess knowledge of the health system, community resources, and volunteer agencies, as well as relevant legislation, standards, and policies.

Compensation
