Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Fraser Health. As a key member of our Home Health Services team, you will provide high-quality patient care and support to clients in their homes and communities.

Key Responsibilities
  • Conduct initial client consultations, assessments, and interventions to determine client needs and develop care plans.
  • Screen referrals and confirm client eligibility for services, ensuring timely and efficient processing of requests.
  • Prepare and maintain accurate client records, providing intake and referral data and statistics as required.
  • Collaborate with interdisciplinary teams to coordinate and prioritize referrals, ensuring continuity of care and optimal client outcomes.
  • Communicate effectively with clients, families, caregivers, and healthcare professionals to provide education, counseling, and guidance on health care resources and services.
  • Perform other related duties as assigned, including maintaining documentation, compiling data, and preparing reports.
Requirements
  • Graduation from an approved program of Practical Nursing, plus one year recent related acute, residential, and/or community-based experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM) and current certificates in CPR Level "C" or equivalent and First Aid.
  • Ability to communicate effectively, both verbally and in writing, and to work independently and in cooperation with others.
