Job Title: Care Coordinator

We are seeking a highly skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.

Key Responsibilities:
  • Assess patient care needs and determine eligibility for services
  • Develop individual care and service plans in collaboration with patients and their families
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources, ensuring patients' values and preferences are respected
Requirements:
  • Membership in good standing with the applicable regulatory body
  • Recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization, and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports, and forms
  • A valid driver's license and access to a reliable vehicle
  • Proficient in a Windows environment
  • Advanced oral and written proficiency in English and in French is essential
