About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at NEXUS Community Support Society.

Key Responsibilities
  • Work collaboratively with individuals and their support networks to develop and implement personalized plans.
  • Facilitate community inclusion and participation in various settings, promoting social connections and a sense of belonging.
  • Conduct regular documentation and record-keeping to track individual progress and identify areas for further support.
  • Participate in meetings, training sessions, and service planning to ensure effective collaboration and communication.
  • Hold a valid Class 5 driver's license and be willing to use a vehicle for work-related purposes.
Requirements
  • Related experience working with adults with diverse abilities is an asset.
  • Basic computer skills and physical ability to perform job duties.
  • First Aid certification (Level 1).
