Job Description

The Northwest Employment Works is seeking a skilled Case Management Specialist to join our team. As a Case Management Specialist, you will be responsible for providing specialized case management services to special needs children identified in the Developmental Services catchment area.

Responsibilities:
• Facilitating access to children's services by providing case management and service coordination for children, youth and their families/caregivers
• Providing general information, determining eligibility, completing intake, prioritizing services using standardized tools, developing initial service plan, referring to appropriate services and tracking client status in CIMS
• Reviewing assessments, diagnosis, plans and recommendations for all assigned clients
• Initiating planning meetings with families and collateral agencies and service partners
• Developing with families and service partners a treatment plan
• Ensuring participation of all service partners in the implementation of the service plan
• Monitoring and documenting client's progress with the treatment plan and evaluating progress of treatment
• Maintaining ongoing communication with family and service providers

Requirements:
• Specialized courses in specific areas of mental health, autism, behavioral or developmental challenges
• University degree in human service field, with two years' experience in the health services environment
• Master's Level Training or equivalent in a related field is an asset
• Experience working with First Nations people and communities
• Experience in case management
• Must be able to maintain confidentiality
• Must have valid Ontario Driver's License, produce a Criminal Records check, and have First Aid & CPR – Adult & Child

Working Conditions:
• This position requires travel to First Nation communities
• The ability to perform the requirements of the position on a regular basis
