We are seeking a skilled Licensed Practical Nurse to join our team at Hearthstone Manor Residential Care Facility. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.
Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients to identify their needs and develop individualized care plans.
- Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring patient safety and comfort.
- Wound Care and Management: Provide wound care and management services, including dressing changes and wound assessments.
- Patient Education: Educate patients and their families on disease management, medication use, and healthy lifestyle habits.
- Documentation and Communication: Maintain accurate and detailed records of patient interactions, treatments, and outcomes, and communicate effectively with healthcare teams and families.
- Licensure and Certification: Current licensure as a Licensed Practical Nurse (LPN) in the province, and certification in CPR and First Aid.
- Education and Experience: Diploma or certificate in Practical Nursing, and at least 1 year of experience in a long-term care or skilled nursing setting.
- Skills and Abilities: Strong assessment and communication skills, ability to work independently and as part of a team, and commitment to providing high-quality patient care.
