Geriatric Nurse Practitioner

We are seeking a highly skilled and experienced Geriatric Nurse Practitioner to join our team at Health Careers in Sask.
About the Role:The successful candidate will be responsible for providing high-quality patient care in a geriatric setting, working collaboratively with a multidisciplinary team to deliver comprehensive care.
Key Responsibilities:- Assess, diagnose, and manage patients with complex geriatric conditions
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare professionals to ensure seamless care transitions and coordination
- Provide education and support to patients and their families to promote healthy aging and disease prevention
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
- Three (3) full-time years of experience as a Registered Nurse
- Bachelor of Science in Nursing (BScN)
- Basic Cardiac Life Support Level C Certificate (BCLS)
- Leadership skills, communication skills, and ability to work cooperatively in an integrated interdisciplinary team
