SchoolsPlus Facilitator

About the Role:
The SchoolsPlus Facilitator will play a key role in implementing a collaborative and integrated model of service delivery to identify and connect students and families to comprehensive services and supports.
Key Responsibilities:
- Work with the SchoolsPlus Leader to bring together multi-sector partners to develop and implement programs and initiatives to meet the needs of school communities
- Coordinate services provided by multiple agencies and government departments for students and families at-risk
- Supervise a team of Community Outreach Workers
- Lead and champion positive ongoing communication with school administration, staff, and a broad range of government and community partners
- Monitor the progress of students served by the program
Qualifications:
- Masters Degree in Social Work preferred or equivalent such as a Bachelors Degree in Social Work with relevant experience
- Eligible for registration with the Nova Scotia College of Social Workers
- Minimum of three (3) years experience managing complex social work cases and facilitating interagency collaboration, preferably serving children and families
- Experience supervising staff
- Valid drivers license and access to a reliable vehicle
