Job Requirements
Master's degree in Social Work from an accredited university
Previous experience relating to patients with cardiovascular disease is an asset
Previous experience in Chronic Disease Management is an asset
Previous experience in Cardiac Rehabilitation setting is an asset
Membership in the Ontario College of Social Workers and Social Service Workers
