We are seeking a highly skilled and dedicated individual to join our team at Island Health as a Social Worker. To be successful in this role, you will require:
- A Bachelor's Degree in Social Work from an accredited university;
- Registration and good standing with the British Columbia College of Social Workers;
- At least two years of experience in a healthcare setting, preferably in Gerontology or Chronic Diseases;
- A working knowledge of healthcare-related legislation and community resources and programs.
An equivalent combination of training and experience may be considered.
Key ResponsibilitiesAs a Social Worker at Island Health, you will be responsible for:
- Completing initial and ongoing client assessments, using clinical and diagnostic reasoning to inform your practice;
- Developing and implementing treatment plans that address the unique needs of each client;
- Providing education and support to clients and their families, both one-on-one and in group settings;
- Integrating best practice and current research evidence into your professional practice decisions and actions;
- Communicating effectively with clients, families, and healthcare teams to ensure seamless care coordination;
- Using critical thinking and problem-solving skills to address complex client needs;
- Promoting client-focused care that demonstrates empathy, compassion, and respect for diverse cultures and preferences;
- Collaborating with interdisciplinary teams to achieve shared goals and objectives;
- Staying up-to-date with industry developments and best practices through ongoing professional development.
