We are seeking a skilled Social Worker to join our team at Island Health. As a Social Worker in our healthcare setting, you will play a vital role in providing high-quality care to our clients.
Key Responsibilities- Conduct initial and ongoing client assessments to identify needs and develop effective care plans.
- Provide teaching and education to clients and their families on various health-related topics.
- Integrate best practices and current research evidence to inform professional practice decisions.
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
- Apply critical thinking and problem-solving skills to resolve complex client issues.
- Collaborate with interdisciplinary teams to promote client-focused care and social justice.
- Develop and maintain strong relationships with clients, families, and community resources.
- Bachelor's Degree in Social Work from an accredited university.
- Registration with the British Columbia College of Social Workers.
- Two years' experience in a healthcare setting, preferably in Gerontology or Chronic Diseases.
- Working knowledge of healthcare-related legislation and community resources.
