Job Summary
We are seeking a highly skilled and experienced Clinical Program Coordinator to join our team at Southlake Regional Health Centre. As a key member of our Cardiac Care Program, you will be responsible for coordinating efficient patient flow activities and ensuring the delivery of high-quality patient care.
Key Responsibilities
- Collaborate with the Triage coordinators and PFN/Charges of the cardiac program to ensure seamless patient flow and efficient use of resources.
- Oversee daily staffing and ensure quality of patient care delivery within the program.
- Lead the team in implementing evidence-based practices that are sensitive to limited resources within a patient-centred care philosophy.
- Collaborate with the manager regarding human resource responsibilities, including recruitment, just-in-time teaching, and ongoing performance evaluation.
- Liaise with members of the inter-professional team to ensure excellence in patient care through monitoring of standards of care and patient outcomes.
- Participate in quality assurance opportunities and support accountability for providing high-quality care.
- Collaborate with the interprofessional teams and PFAs to create the ultimate patient experience.
Qualifications
- RN Certificate of registration from the College of Nurses of Ontario.
- Baccalaureate Degree in Nursing Science (preferred).
- Minimum of 2-5 years recent and relevant cardiac care experience.
- CNO, RNAO, other professional affiliations as appropriate.
- Demonstrates knowledge in clinical practice in the Cardiac Program.
- Demonstrates the ability to communicate and work with all members of the interprofessional team.
- Demonstrates the ability to problem-solve clinical issues with the support of the manager.
- Demonstrates a strong patient and customer-focused philosophy in all interactions.
- Possesses the ability to implement strategies that effectively facilitate change.
- Demonstrates clinical decision-making and organizational skills.
- Demonstrated excellence in communication, both written and verbal.
- Demonstrated problem-assessment skills.
- Demonstrated commitment to customer service and teamwork.
- Demonstrated individual leadership skills.
