The OpportunityThe Social Worker assists residents and their families by providing support and assessment for residents, related to move-in process, transitions, finances, concerns, and decline in health, as well as assistance accessing community resources.
Key Responsibilities- Assist the Director of Care and Business Manager with the process of moving new residents into the home by coordinating with incoming residents and their Power of Attorney (POA) and providing support with aspects of the move-in process that require the involvement of the Social Service Worker.
- Participate in the residence's Continuous Quality Improvement (CQI) program.
- Evaluate the effectiveness of Social Service Work services on an annual basis and provide input to update policies and procedures that impact the Social Service Work practice.
- Prepare and submit reports to the residence and/or corporate office as required.
- Bachelor's or Master's Degree in social work.
- Minimum one-year relevant experience preferably in a healthcare setting.
- Previous experience in a long-term care setting is preferred.
