Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a Social Worker, you will play a vital role in providing therapeutic clinical counselling services to patients and families in our assigned clinical area.

Key Responsibilities
  • Conduct social work assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices.
  • Provide consultation to patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice relevant to the assigned clinical area.
  • Maintain patient records by documenting patient assessments, charting patient information, and preparing progress notes and reports in accordance with established standards, policies, and procedures.
  • Provide leadership in the discharge planning process by interviewing patients and families to assess and determine care plans and types of services required.
  • Communicate with community health and social agencies, including government ministries, Home Health personnel, and family physicians, to collaborate in the assessment and management of individuals at risk.
Requirements
  • Bachelor's Degree of Social Work from an accredited School of Social Work.
  • One year's recent, related experience in a hospital setting or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
Skills and Abilities
  • Comprehensive knowledge and understanding of emotional and mental health issues related to illness and injury, and patient/family cultural dynamics.
  • Basic counselling skills, including crisis counselling, brief therapeutic interventions, and loss and grief applicable to families, young adults, and seniors.
  • Ability to provide consultation to patients, families, and healthcare professionals.
  • Ability to develop, coordinate, and deliver in-services, presentations, and support groups.
  • Ability to provide leadership to the interdisciplinary team in care planning for the effective utilization of patient resources.
