Your Opportunity:
We are seeking a highly skilled Nurse Practitioner to join our Addiction and Mental Health Outreach Team in Medicine Hat, Alberta. As a key member of our multi-disciplinary team, you will provide comprehensive health services to individuals across their lifespan, with a focus on patient-centered, trauma-informed, and recovery-based care.
About the Role:
As a Nurse Practitioner, you will be responsible for assessing, diagnosing, and managing common and complex health conditions, promoting health, and preventing illness/injury. You will work collaboratively with community partner teams to provide care to individuals who require treatment for a variety of medical conditions, including chronic disease, acute wound care, and STBBI testing/treatment.
Key Responsibilities:
- Provide comprehensive health services to individuals across their lifespan
- Assess, diagnose, and manage common and complex health conditions
- Work collaboratively with community partner teams to provide care to individuals who require treatment
- Develop and implement individualized care plans to promote health and prevent illness/injury
- Order and interpret diagnostic tests, prescribe treatment, and perform procedures as needed
Requirements:
- Successful completion of an approved Nurse Practitioner program and a minimum of master's degree
- Registration with CRNA on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
- Knowledge and/or experience of addiction and/or mental health treatment modalities
- Proven ability to work with community stakeholders and foster positive relationships within a multidisciplinary team environment
Preferred Qualifications:
- Experience providing Addiction and Mental Health/Public Health services
- Experience engaging complex clients in a variety of settings
- Knowledge and/or experience of harm reduction strategies
- Knowledge of testing and treatment pathways for HIV and HCV
