Patient Care Coordinator

We are seeking a highly skilled and experienced Patient Care Coordinator to join our team at Burnaby Hospital. As a key member of our healthcare team, you will be responsible for providing clinical supervision, guidance, and support to nursing staff and other clinical team members.
Key Responsibilities- Apply established criteria and prioritize admissions/intake, lead and facilitate actions related to daily patient flow, plan for patient discharge, and appropriate patient placement/referral.
- Supervise nursing and nursing support staff, coordinate the work of the clinical team, and monitor the quality of care and the safety of patients.
- Support quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education, and acting as a mentor and resource to staff.
- Initiate/lead/support plan of care conferences and rounds for the purpose of optimal patient care management according to best practice.
- Identify learning requirements of staff, provide input into the development of learning objectives, and provide clinical support and guidance.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience.
- Completion of post-graduate coursework and/or certification applicable to the designated clinical area.
