Social Services Specialist

We are seeking a highly skilled Social Worker III to join our team at the Texas Department of Aging & Disability Services. As a key member of our interdisciplinary team, you will be responsible for providing complex social services, including liaison work between patients, legally authorized representatives, and other stakeholders. Your expertise will be instrumental in ensuring ongoing care, treatment, and support for individuals with mental health needs.
Key Responsibilities:
Conduct thorough assessments of individuals' support systems and service needs, including participation in interdisciplinary team meetings as needed.
Design and provide individual and group trainings to promote the exercise of rights and patient transition, including curriculum development and case-specific information.
Assist with the coordination of furloughs, transfers, transitions, and discharges, ensuring a smooth and orderly process.
Focus on complex cases, concentrating on post-discharge placement and support.
Work under limited supervision, with considerable latitude for the use of initiative and independent judgment.
Requirements:
Master's degree in Social Work or related field.
Knowledge of human growth and development, behavioral and mental health, and social work standards for case management.
Ability to communicate effectively in writing and verbally in both individual and group settings.
Skill and ability to assign and supervise the work of others.
Ability to perform all job duties in accordance with the Texas State Board of Social Work Examiners Code of Conduct and National Association of Social Workers Code of Ethics.

Essential Job Functions:
Attends work on a regular basis and may be asked to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Knowledge Skills Abilities:
Knowledge of human growth and development, behavioral and mental health, developmental and emotional components of physical illness, individuals with intellectual and developmental disabilities, social work standards for case management; facility and community resources, and the principles, methods and theories used to practice as a social worker.
Ability to communicate effectively in writing and verbally in both individual and group settings.
Skill and ability to assign and supervise the work of others.
Skill in accountability for assigned property with limited financial responsibilities.
Ability to perform all job duties in accordance with the Texas State Board of Social Work Examiners Code of Conduct, National Association of Social Workers Code of Ethics and in compliance with state, state hospital policies and procedures.
