About the Role:

The successful candidate will work collaboratively with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, and group work, system navigation and advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management.

Key Responsibilities:

  • Provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.
  • Work collaboratively with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, and group work.
  • Facilitate client self-management and provide education on relevant health and social issues.
  • Develop and maintain relationships with community resources and services to support clients.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • Master's degree in Social Work or a related field.
  • Registration with the British Columbia College of Social Workers.
  • Experience working in a healthcare setting, preferably in a Primary Care Network.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
