About the Role

The Public Health Nurse (PHN) is a key member of the Public Health Services team, providing nursing services within the framework of the Population Health/Health Promotion model. This role involves translating knowledge from the health and social sciences to support health-enhancing behaviors among diverse individuals, families, and population groups through universal and targeted interventions, programs, and advocacy.

Key Responsibilities

• Provide nursing services in communities, schools, homes, and outreach locations to individuals, families, and communities.
• Offer services that promote health, prevent disease, and protect populations from health risks.
• Collaborate with other healthcare professionals to develop and implement health promotion programs and policies.
