Nurse Practitioner

We are seeking a highly skilled Registered Nurse to join our Cardiac Services team at Fraser Health. As a key member of our interprofessional care team, you will provide direct patient care to patients in an acute medical unit, working collaboratively to provide person-centered care.
Key Responsibilities- Assess, plan, implement, and evaluate nursing care for assigned patients, identifying actual or potential problems and developing interventions to address them.
- Collect information from various sources, using skills of observation, communication, data review, and physical assessment to inform nursing decisions.
- Coordinate and provide nursing care to assigned patients, ensuring that care is delivered in accordance with physician and anaesthetist orders and established standards and hospital policies.
- Set priorities, assign, and delegate tasks to appropriate staff, modifying care as needs change.
- Initiate immediate treatment, such as applying non-invasive monitoring devices as required.
- Explain procedures to patients and/or teach patients how to deal with the results of procedures.
- Obtain, check, set up, and position supplies, instruments, and equipment required for interventional procedures, ensuring that instruments and equipment are in working order.
- Report instruments/equipment in need of repair.
- Perform circulation duties, including passing instruments and supplies, medications, and other materials to the team.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Collaborate with members of the interprofessional care team, including physicians, licensed practical nurses, patient care coordinators, clinical nurse educators, and other healthcare professionals/providers, to identify and resolve issues, adjust care plans as required, and address changes in patient condition.
- Coordinate nursing care for patients, setting priorities, assigning, and delegating tasks to support staff, and making referrals to the interprofessional care team according to established protocols and practice standards.
- Collaborate with patients and their families/caregivers in developing care plans, encouraging family participation in the development, implementation, and ongoing modification of care plans.
- Establish a therapeutic relationship by demonstrating empathy, trust, and respect, advocating for patients' rights to privacy, dignity, and access to information, and ensuring patients' choice and autonomy in decision-making and care planning.
- Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to help patients accomplish their healthcare goals.
- Assess learning needs of patients and families/caregivers, providing information and demonstration of care activities to support self-care and the discharge plan.
- Plan and discuss referral information with patients and their families/caregivers, and, in collaboration with members of the interprofessional care team, initiate referrals to appropriate healthcare/community agencies.
- Dismantle tables in accordance with care and handling instructions of special instruments and Occupational Health and Safety regulations.
- Participate in nursing committees, care conferences, and councils as assigned or as agreed to with the Manager.
- Participate in research and quality improvement activities by identifying patient care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures, or protocols.
- Participate in and attend in-services and other educational programs.
- Identify own learning goals and maintain and update current clinical competence and knowledge within the area of practice.
- Act as a preceptor or mentor to assigned new nursing staff and students, sharing information, demonstrating care activities, and participating in orientation.
- Provide feedback to the Manager/delegate regarding the effectiveness of patient care programs, policy/clinical decision support tools, and equipment.
- Act as an advocate to protect and promote patients' rights to autonomy, respect, privacy, dignity, and access to information.
- Participate as an active member on nursing and hospital committees and councils for improvement of patient care and/or work environment.
- Participate in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor and mentor as appropriate.
- Participate in the evaluation of patient care programs, techniques, standards, and procedures by providing feedback about effectiveness and patient care outcomes.
- Maintain a safe environment for patients, families, and staff by adhering to established safety and emergency practices and procedures.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Completion of Cardiology component of Post Basic Critical Care course, or post basic High Acuity Course, approved Arrhythmia interpretation course plus one year's recent experience in cardiac monitoring or telemetry unit, or an equivalent combination of education, training, and experience.
- Current CPR (Level C) Basic Rescuer Certificate.
- Clinical proficiency in the designated clinical area(s).
- Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions.
- Ability to determine and deal with emergency situations.
- Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the interprofessional care team using verbal, written, and electronic communication means.
- Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy.
- Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
- Ability to teach and instruct patients and families/caregivers about topics essential to their healthcare and well-being.
- Ability to organize and prioritize work.
- Ability to collaborate with and promote cooperation among members of the interprofessional care team.
- Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
- Ability to operate related equipment, including relevant computer applications.
- Physical ability to perform the duties of the position.
