Fraser Health is seeking a highly skilled and experienced Registered Nurse to fill the role of Patient Care Coordinator. This position is responsible for providing clinical supervision, guidance, and support to nursing staff, other clinical team members, and support staff within the assigned unit(s). The successful candidate will have a strong background in patient flow management, quality improvement, and risk management.
Key Responsibilities- Develop and implement effective patient flow plans to ensure optimal patient care and efficient use of resources.
- Provide clinical supervision and guidance to nursing staff, including mentoring, coaching, and performance management.
- Collaborate with the clinical team to identify and address quality improvement opportunities and implement evidence-based practices.
- Participate in research and special projects to improve patient care and outcomes.
- Develop and maintain relationships with internal and external stakeholders to ensure effective communication and collaboration.
To be successful in this role, you will require:
- A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience.
- Completion of post-graduate coursework and/or certification applicable to the designated clinical area.
- Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment.
- Ability to prioritize tasks, manage multiple projects, and meet deadlines.
