Pediatric Palliative Care Specialist

We are seeking a highly skilled Pediatric Palliative Care Specialist to join our team at Canuck Place Children's Hospice. As a key member of our interdisciplinary team, you will provide comprehensive care to children with serious illnesses and complex conditions, and their families, across the healthcare continuum.

Key Responsibilities:

  • Diagnose and treat illnesses and conditions within your scope of practice, order appropriate screening and diagnostic investigations, interpret reports of investigations, and analyze information to monitor progress and plan treatment.
  • Prescribe treatment and medications within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
  • Provide direct care and monitor ongoing care in the hospice and in families' homes.
  • Participate in patient rounds and collaborate with team members to develop and implement care plans.
  • Establish priorities for management of health conditions, provide expertise in pain and symptom management, and ongoing care needs of children and families, support discussions about serious illness, and engage in advance care planning.
  • Promote ethical decision-making and child/family advocacy.
  • Act as an advocate, clinical resource, consultant, and role model for the provision of content, holistic, and ethical pediatric palliative care for Canuck Place children and families.
  • Provide consultation and education to inter-professional healthcare providers, community groups, and healthcare institutions regarding pediatric palliative care.
  • Collaborate and coordinate to facilitate complex care at transition points, including admissions/transfers between hospice, hospital, and home, and collaborate with other community partners as needed to provide continuity of care based on the family's goals of care in conjunction with best practices of pediatric palliative care.
  • Maintain documentation of patient assessment, treatment, and discussions, and communicate information to team members and providers.
  • Provide input into the development, implementation, and evaluation of organizational and departmental philosophy, objectives, policies, and procedures to ensure appropriate levels of care and cost-effective use of resources.
  • Promote excellence in clinical practice and patient safety through the application of clinical expertise and principles of continuous quality improvement, evaluate and analyze the appropriateness of research for use in the clinical setting, and participate and promote dissemination of research findings and knowledge translation.
  • Understand and apply the concepts of outcomes research, identify research needs within the program or larger pediatric palliative care specialty, collaborate with physicians, advanced practice nurses, and research teams to develop research activities, and lead or participate in research projects as appropriate.

Requirements:

  • Successful completion of an approved Nurse Practitioner program with a Master's of Science in Nursing (MSN).
  • Current or qualifies for registration as a Nurse Practitioner with the BC College of Nurses and Midwives (BCCNM) and have completed or be eligible to complete the Objective Structured Clinical Examination (OSCE).
  • Minimum 5 years' experience in pediatric care and/or palliative care.

  • Able to critically assess and evaluate health research literature to determine best practices, ability to introduce education and evidence-based research into practice areas.
  • Demonstrated ability to plan, organize, set, and successfully accomplish objectives.
  • Able to define and articulate the specific areas of practice and client population for whom the NP is competent to provide healthcare services.
  • Strong computer skills, including MS Office suite programs.
  • Self-identifies learning needs and is a continuous, lifelong learner.
  • Experience and comfort working independently and as part of a larger team, as well as in a hospice, hospital, and home environments.
  • Physical ability to do the duties of the position and ability to operate related equipment.
