Job Summary

We are seeking a compassionate and dedicated Caregiver to join our team at Extendicare Canada Inc. as a Personal Support Worker (PSW). The successful candidate will provide direct personal support and care to patients in accordance with established policies, procedures, and work routines.

Key Responsibilities

  • Assist patients with daily living activities such as ambulation, feeding, bathing, and grooming.
  • Provide emotional, social, and spiritual support to patients and their families.
  • Collaborate with healthcare teams to develop and implement care plans.
  • Document patient care and progress in accordance with established protocols.
  • Participate in ongoing education and training to enhance skills and knowledge.

Requirements

  • Completion of a PSW program or HCA/CCA certification.
  • Experience in long-term care or community home care settings, preferably with exposure to dementia and palliative care.
  • Knowledge of care planning systems and documentation.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Working Conditions

The successful candidate will work in a fast-paced, dynamic environment with a focus on patient-centered care. A reliable vehicle is required for community home care assignments.
