About the Role
- Develop and maintain effective Care Plans with respect to Community Health Services, independently or in cooperation with the Home and Community Care Support Services (HCCSS).
- Work with patients to provide exceptional nursing services, both in-person and virtually.
- Provide services in various settings, including in-home, clinic, retirement home, or school.
- Communicate and liaise with patients, family members, and other healthcare professionals.

Requirements
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Exceptional interpersonal skills and a strong desire to make a difference
- A valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including a 2-step TB test
