Compassionate Care Coordinator

Job OverviewWe are seeking a highly skilled and compassionate individual to join our team as a Compassionate Care Coordinator. In this role, you will work closely with our clients and their families to develop personalized care plans that meet their unique needs and goals.
Key Responsibilities- Develop and implement comprehensive care plans for clients, taking into account their physical, emotional, and social needs
- Provide ongoing support and guidance to clients and their families, ensuring they have access to the resources and services they need
- Work collaboratively with healthcare professionals and other stakeholders to ensure seamless coordination of care
- Conduct thorough assessments of client needs, identifying areas where additional support may be required
- Develop and maintain relationships with community partners and service providers to ensure clients have access to a range of services and supports
- Regulated health care professional with registration as a health or social work professional, including registered nurse, physiotherapist, occupational therapist, speech language pathologist, or social worker
- Minimum of one year of experience within the community support service sector or healthcare environment
- Proven experience working with vulnerable populations, including the frail and elderly
- Strong knowledge of community resources, including funding sources and supportive services
- Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders
This is a full-time position, requiring a valid Ontario driver's license and access to a reliable vehicle. On-call duties are required, and employees must be fully vaccinated against COVID-19.
The estimated salary for this position is $60,000 - $80,000 per annum, depending on experience.
