About the Job

We are seeking a full-time Licensed Practical Nurse (LPN) to join our team in Rutland, VT. As a hospice LPN, you will provide individualized goal-directed nursing care through the use of the nursing process in accordance with physician's orders.

  • Provides quality care to assigned patients.
  • Carries out the plan of care for each patient, ensuring that the patient/caregivers are included in the care plan process.
  • Ensures documentation is completed, communicating via a note the activities and needed actions of each visit to the RN.

The ideal candidate will have prior clinical acute care nursing experience, a valid Driver's License, Vehicle, and Insurance, and be able to successfully pass motor vehicle record checks, child abuse, criminal record checks, and adult abuse checks.

Job Responsibilities

As a hospice LPN, your responsibilities will include:

  1. Providing quality care to assigned patients.
  2. Implementing medical plans of care, nursing interventions, and procedures as necessary to ensure safe, effective, and quality care for patients.
  3. Evaluating aspects of implemented plans of care, including identified problems, care provided, and patient responses, to meet patient/caregiver needs and ensure overall quality of care delivered.
