Job Summary:

The successful candidate will be responsible for providing nursing care to patients in a long-term care setting. This will include assessing patients' needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities:
  • Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Provide direct patient care, including administering medications, performing treatments, and providing emotional support.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care delivery.
  • Document patient progress and communicate with other healthcare professionals to ensure continuity of care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Current registration with the College of Nurses of Ontario.
  • Experience working in a long-term care setting, preferably in a geriatric or palliative care environment.
