Social Worker I, Acute Care Specialist

We are seeking a highly skilled and compassionate Social Worker to join our Acute Care team at Fraser Health Authority. As a Social Worker, you will play a vital role in providing comprehensive therapeutic clinical counselling services to patients and families in our hospital setting.
Responsibilities:- Conduct comprehensive social work and psycho-social assessments of patients and families to identify problems, personal concerns, and treatment objectives.
- Provide grief and bereavement support to patients and families, and facilitate change in coping style, behavior, attitude, and feelings as it relates to the illness and other system issues.
- Educate and provide consultation to patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice relevant to the assigned clinical specialty area.
- Maintain patient records by documenting patient assessments, charting patient information, and preparing progress notes and reports in accordance with established standards, policies, and procedures.
- Provide leadership to the interdisciplinary team in care planning, including discharge planning and hospital stay, and collaborate with hospital and community services to determine alternate levels of care and the effective utilization of patient resources.
- Masters Degree of Social Work from an accredited school of Social Work.
- Current full registration with the BC College of Social Workers.
- Ability to initiate, develop, and carry out research activities.
- Demonstrated ability to communicate effectively both verbally and in writing with patients, families, staff, and community agency personnel.
- Ability to operate related equipment.
