Social Worker I

We are seeking a highly skilled and compassionate Social Worker I to join our Acute Care team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive therapeutic clinical counselling services to patients and families in our Acute Care setting.
Key Responsibilities- Conduct comprehensive social work and psycho-social assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices.
- Provide leadership to the interdisciplinary team in care planning, including discharge planning and hospital stay, to ensure effective utilization of patient resources.
- Communicate with community health and social agencies to facilitate referrals and access to community resources.
- Act as a resource to patients and their families, providing information and support to enhance their health and well-being.
- Participate in local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery.
- Masters Degree of Social Work from an accredited school of Social Work.
- Current full registration with the BC College of Social Workers.
- Ability to initiate, develop, and carry out research activities.
- Demonstrated ability to communicate effectively with patients, families, staff, and community agency personnel.
