Position Summary

We are seeking a skilled Social Worker II to provide comprehensive psychosocial care and support to clients and their families in a healthcare setting. The ideal candidate will have a strong commitment to the care and well-being of others and be able to work collaboratively with the multidisciplinary team to provide direct care, counseling, and group work.

Key Responsibilities:

• Provide comprehensive assessments and care planning for designated clients and their families
• Develop and implement treatment plans to address the social and emotional needs of clients
• Collaborate with other healthcare providers to provide coordinated care and support
• Provide direct psychosocial care, counseling, and group work to clients and their families
• Facilitate client self-management and provide education and linkage to relevant resources

Requirements:

• Bachelor's Degree in Social Work from an accredited university
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders
• Current registration with the British Columbia College of Social Workers
