Overview
Home and Community Care Support Services is seeking a skilled Registered Nurse to join our team as a Rapid Response Nurse. This is a casual position that requires flexibility and a commitment to providing high-quality patient care.
Key Responsibilities
- Conduct thorough assessments of patients in their homes to identify their needs and develop individualized care plans.
- Collaborate with healthcare teams to ensure seamless transitions of care and prevent hospital readmissions.
- Provide education and support to patients and their families to promote self-management of chronic conditions and prevent complications.
- Communicate effectively with patients, families, and healthcare providers to ensure that care is coordinated and patient-centered.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
- Current registration with the College of Nurses of Ontario.
- Minimum 5 years of experience as a Registered Nurse, preferably in a community or gerontological setting.
- Advanced education in gerontology and/or chronic disease management is an asset.
- Excellent assessment, communication, and problem-solving skills.
- Ability to work independently and as part of a team.
- Flexibility to work a variety of shifts, including evenings and weekends.
