Make a Difference in Mental Health

As a Therapist, Mental Health & Addiction, you'll be part of a collaborative team dedicated to providing seamless services and programs to our clients and customers.

Key Responsibilities:
  • Coordinate and conduct assessments to guide treatment planning
  • Provide case management services, including service planning, coordination, navigation, and evaluation of progress
  • Deliver individual and group psychotherapy using evidence-informed practices, including Motivational Interviewing, Stage-Matched Intervention, and Trauma-Informed approaches
  • Offer individual and group support to family members and lead group counseling and education sessions
  • Collaborate with physicians and the clinical team to monitor patient progress and create treatment plans
  • Develop and implement discharge planning and referrals to community resources
  • Identify discharge needs based on comprehensive patient assessments and care plans
  • Provide service and team leadership by establishing treatment goals and working with patients and team members to develop action steps
  • Lead team monitoring and evaluation of progress on treatment plans
  • Ensure effective communication among patients, teams, and programs
  • Complete documentation and record-keeping as per Homewood Health Centre and governing College policies
Requirements:
  • Bachelor's or master's degree in a recognized Social Science program with addiction-specific education
  • Membership in good standing with the relevant regulatory college/governing body in Ontario or pursuit of relevant credential
  • Competence in providing CBT, DBT, and other evidence-based approaches for treating complex mental health conditions and substance use disorders
  • Demonstrated skills and experience in assessment and treatment using evidence-based practice
  • Exceptional communication and interpersonal skills
  • Awareness and acknowledgement of the value of using measurement-based care to support patients/clients
  • Ability to facilitate and lead an interdisciplinary team, orchestrating team resources
  • Individual and group counseling skills, including presenting psycho-education to large groups
  • Capacity to deliver inclusive and culturally responsive approaches
  • Knowledge of addictions and concurrent issues along a continuum of care
  • Valid Ontario driver's license and access to own vehicle where required
  • Knowledge of the Mental Health Act, PHIPA, Ontarians with Disabilities Act 2005, and Health Care Consent Act
