About the Role:

FreshCo is seeking a highly skilled and dedicated Staff Pharmacist to join our team in Medicine Hat, Alberta. As a key member of our pharmacy team, you will be responsible for providing exceptional patient-centered care and exceeding patients' expectations.

Key Responsibilities:

  • Patient Care:
    • Provide clinical services as appropriate to practicing top of scope in your province, including chronic health condition monitoring and management, medication reviews, health risk assessments, prescription renewals or modifications, minor ailments, or other prescribing services.
    • Initiate care plans, follow up, and complete associated documentation.
    • Collect relevant patient, drug, and medical condition information.
    • Identify and resolve actual and potential drug-related problems.
    • Monitor and optimize medication adherence.
    • Provide patient counseling that is tailored to the needs and concerns of the patient to ensure comprehensive understanding of the medication.
  • Customer Service:
    • Provide superior service and exceed patient expectations.
    • Acknowledge patients promptly and professionally with warmth, empathy, and compassion.

Requirements:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy from a recognized institution.
  • Licensed Pharmacist in the province of responsibility.
  • Authorization to Administer Injections.
  • Excellent attention to detail, ability to multi-task, and work independently in a fast-paced environment.
