Job Title: Social Worker

Medavie Health Services is a national leader in primary health care solutions, delivering innovative community paramedicine programs and mobile integrated health services across Canada.

As a Social Worker, you will be part of a compassionate network of 4,300 health care professionals in six provinces, working together to provide high-quality patient care through ground and air ambulance services, medical communications, and mobile integrated health/community paramedicine programs.

Our team is dedicated to building a healthier Canada and supporting the needs of culturally diverse communities. We are seeking a skilled Social Worker to join our team in Listowel, Ontario.

Key Responsibilities:
  • Conduct in-home client visits with the Mobile Mental Health Team.
  • Provide client support through Tele-Support access line.
  • Assess client's condition and presenting problem, and contributing factors.
  • Collaborate with law enforcement and/or Paramedic Services.
  • Recommend and facilitate referrals to appropriate alternative resources.
  • Employ a technical knowledge of psychotropic medication.
  • Gather information from interviews with the client and collateral information from family and other professionals.
  • Use substantive knowledge of mental illness, cognitive functioning, medical co-morbidity, and substance abuse to formulate clinical impressions and recommendations.
Requirements:
  • Considerable clinical experience in mental health settings and community work.
  • Registration and good standing with the Ontario College of Nurses.
  • Ability to provide initial psychological interviews and assessments.
  • Experience on interdisciplinary teams, in program development, and case management.
  • Independent critical thinking, clinical judgment, and decision-making skills.
  • Valid Driver's license in good standing.
  • Good previous work attendance record.
