We are seeking a highly skilled and compassionate Clinical Social Worker to join our Acute Care team at Fraser Health Authority. As a key member of our interdisciplinary team, you will provide expert social work services to patients and families in our acute care setting.
Key Responsibilities- Conduct comprehensive psychosocial assessments of patients and families to identify needs and develop effective care plans.
- Provide individual and group counseling services to patients and families, addressing emotional and mental health issues related to illness and injury.
- Collaborate with healthcare professionals to coordinate services, manage cases, and facilitate meetings.
- Develop and implement discharge plans, ensuring a smooth transition for patients and families.
- Provide education and support to patients, families, and healthcare professionals on topics such as coping with illness, grief, and loss.
- Participate in research activities and program planning, contributing to the development of evidence-based practices.
- Maintain accurate and comprehensive patient records, adhering to established standards and policies.
- Bachelor's Degree in Social Work from an accredited institution.
- Current full registration with the BC College of Social Workers.
- Minimum one year of experience in a hospital setting or equivalent combination of education and experience.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multicultural environment.
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
