About the Role

We are seeking a compassionate and dedicated Personal Support Worker to join our team at Transitions Care, Inc. as a Nursing Care Specialist. In this role, you will provide high-quality care and support to patients in their homes, ensuring their comfort, safety, and well-being.

Key Responsibilities
  • Accompany patients on outdoor recreational activities to promote physical and mental well-being
  • Assist with cleaning, sterilizing, and setting up medical equipment to maintain a safe and healthy environment
  • Perform other duties related to patient care and comfort, including bathing, dressing, and grooming
  • Take patients' vital signs, such as blood pressure, temperature, and pulse, to monitor their health
  • Deliver messages, reports, and specimens between departments to ensure seamless communication
  • Make beds and maintain patients' rooms to promote a clean and comfortable living space
  • Supervise patients' exercise routines to promote physical activity and mobility
  • Maintain inventory of supplies to ensure adequate stock and prevent waste
Requirements
  • Secondary (high) school graduation certificate
  • First Aid Certificate and CPR Certificate
  • Ability to work in a home care and visiting care agency setting
  • Ability to work in various locations, including patients' homes
  • Ability to lift, turn, and position patients safely
